	Initial Phone Consultation In our complementary initial conversation, we will explore the situation with you and isolate the issues that need to be addressed and help you set priorities for action.
	Assessment Our comprehensive review of medical history and analysis of current medical & cognitive issues results in a specific care plan for your elder loved one.
	Implementation We provide you with a complete written assessment report. We draw a line in the sand explaining exactly where the situation is currently - and illustrating the care plan to match the elder's needs with the elder care services available in the community. We then assist the elder and/or family with obtaining these services.
	Monitoring & Safety Net Your RN Care Manager is your "nurse in the family," who will be a constant source of support and information for both the elder and the extended family. We are on the spot if an urgent need for support arises.
