Advocare’s Director of Life Care Planning Jill Poser has completed the Geriatric Care Management certification course from the University of Florida and holds a Bachelor of Arts from SUNY Oneonta. Jill’s vision of an integrated care management model is the result of over thirty years of business experience including accessibility construction and renovation, care management training, Medicare home health, and business development. As Director of Life Care Planning, Jill places special emphasis on non-medical issues and the challenges created by long life and/or disability. She quarterbacks an inter-disciplinary team of professionals who work together to identify present and potential long term care needs, synchronizing the client’s financial resources and legal services with ongoing care plans. She specializes in the areas of transition, placement, skilled nursing and entitlement programs.
Jill is both a Certified Aging-in-Place Specialist and Certified Geriatric Care Manager. She is devoted to educating the community-at-large and was a featured speaker at the Alzheimer Community Care Educational Conference, the National PCA Conference in Washington D.C., FAU Memory & Wellness Center, Attorney/Care Management Elder Concert at FAU, and the FICPA Academy Conference in Tampa, FL. She proudly serves on the A.W. Dreyfoos School of the Arts Advisory Council, Jewish Federation of South Palm Beach County Business & Professional Cabinet, and is co-founder of the ‘Times of Your Life’ charity gala for the Southeast chapter of the Alzheimer’s Association.
